Introduction: The discussion from the context of “region” notifies commuting in the context of public health as a determinant in the health-disease process. The study raised the hypothesis of the existence of a pendular migration flow in search of the Hemodialysis service in the state of Maranhão between 2011 and 2016. secondary objectives, to trace and spatialize the possible paths and profiles of patients. Method: This is a quantitative, ecological study with an observational analytical model. A descriptive statistical study, from a secondary data source, with quantitative variables and simple random sampling, was established, with the objective of determining the means of the variables. The research character was inductive. Excel 16.0, EPI INFO 7.2, Google Earth Pro 7.3, Qgis 3.14 and Nefrodata software were used. Of the 269 forms, 263 were selected based on the inclusion and exclusion criteria. Results and discussion: Through the mapping of cases, it was found that 17.55% of patients performed pendulum migration and those from the macro-region of Pinheiro represented the highest number, with an average of 26.08%. There was a predominance of females (54.35%); browns (76.74%); incomplete elementary education (75.86%); age group from 5 to 14 (41.30%); and 6 registered deaths (75%). Final Considerations: There was pendulum migration in search of hemodialysis services in the state of Maranhão. Geographic accessibility to health services has been proven as a relevant determinant in treatment adherence.
