Digital care stratification for hypertension based on the Chronic Conditions Care Model (MACC): development and pilot evaluation

This study developed and preliminarily evaluated a digital tool based on the Chronic Care Model (MACC) to support care stratification and care planning for people with hypertension in Primary Health Care. This was a technology-development study with a pilot evaluation in a Family Health Strategy unit in Recife, Brazil. Among 517 registered people with hypertension, 60 eligible records were selected by simple random sampling. The deterministic algorithm assigned care levels 3, 4 and 5 and generated a care plan. Ten clinical cases were used for consistency testing and 48 participants aged 40-75 years were compared with HEARTS. The tool reproduced the clinical reference classification in all 10 test cases. In the pilot sample, 36 (60.0%) were level 3, 18 (30.0%) level 4 and six (10.0%) level 5. Agreement with the operational HEARTS categorization was 79.2% (38/48), with kappa=0.65 (weighted=0.78). The tool was technically feasible for organizing care but is not a validated prognostic model. External and prospective studies are required before clinical impact can be inferred.
