Where do Brazilians buy food? Temporal trends of food retail outlet prevalence

Abstract

Introduction: Food retail outlets are central components of food environments, influencing population diet quality. In Brazil, previous studies focused on the caloric share or grams of purchased foods, but not on the frequency and diversity of outlets. Objective: To describe temporal trends in the prevalence and diversity of food retail outlets used by Brazilian households between 2002 and 2018, and to compare the concomitance of these outlets in 2002-2003 and 2017-2018. Methods: Data were obtained from the Brazilian Household Budget Surveys (HBS) conducted by the Brazilian Institute of Geography and Statistics in 2002-2003, 2008-2009, and 2017-2018. Food retail outlets were grouped into ten categories: supermarkets; small markets/grocery stores; street fairs and produce stalls; food supply centers; bakeries and confectioneries; small producers; butcher shops, poultry shops and fish markets; street vendors; bars, coffee shops and restaurants; and others. We analyzed the prevalence (mean percentage) and the mean number of outlets (diversity) used within seven days at the national level, by area, region, and income quintiles between 2002 and 2018. For concomitance analysis, only national-level data from 2002-2003 and 2017-2018 were considered. Results: The prevalence of purchases in supermarkets increased from 54.8% (2002-2003) to 64.5% (2017-2018), while purchases at street fairs and produce stalls decreased from 36.6% to 20.9%, and at bakeries and confectioneries from 50.5% to 37.5%. The mean diversity of outlets visited weekly declined from 2.8 to 1.9. This pattern was consistent across areas, regions and income quintiles. In the comparison between 2002–2003 and 2017–2018, concomitance analysis showed a growing concentration of purchases in supermarkets. Conclusion: Between 2002 and 2018, Brazil’s food retail environment became increasingly concentrated in supermarkets and less diverse, alongside the decline of traditional outlets. These structural changes require attention from public food and nutrition policies.
