In recent years, saliva has been shown to possess biomarkers capable of diagnosing different diseases, including gastric cancer. Here we present a systematic review (PROSPERO CRD42021259519) with the aim of evaluating biomarkers closer to clinical use. We identified markers of different molecular nature, of which 3 proteins stand out (CSTB, TPI1 and DMBT1), which in the validation stage presented high diagnostic accuracy. Salivary biomarkers have great potential for the detection of stomach cancer; however, it is recommended in subsequent studies to improve the design and execution phases of the research in order to obtain better quality evidence.
