A Psychology Without a Couch: Effects of Sustained Contemplative Practice in Tibetan Buddhism on Neuroplasticity and Psychological Processes. A Systematic Review

Abstract

Background: Sustained or intensive meditation practices from Tibetan Buddhism constitute a rigorous system of mental training and a sophisticated "operational psychology." While empirical evidence suggests their capacity to induce beneficial transformations in psychological processes and the brain, it had not been synthesized comprehensively and critically.

Objective: To analyze the empirical evidence on the effects of sustained or intensive Tibetan meditation in adults, examining key psychological processes (emotional regulation, cognitive control, self-referential processing, meta-awareness) and their neuroplasticity correlates.

Methods: A systematic review was conducted following the PRISMA 2020 guidelines across five databases (PubMed, PsycINFO, ERIC, SciELO, LILACS), identifying empirical studies on practices such as śamatha, vipassana, and compassion methods in contexts of expertise or intensive retreats.

Results: Out of 354 records, 33 studies were included. The synthesis revealed a convergent pattern organized into three interconnected domains: 1) reconfiguration of the Default Mode Network (reduced activation and greater connectivity with control networks), 2) strengthening of cognitive and attentional control with greater neural efficiency, and 3) transformation of affective regulation and cultivation of prosocial qualities. Structural (insula, hippocampus) and functional changes underpinning these improvements were documented.

Conclusion: The findings strongly support that sustained and intensive Tibetan meditation acts as a highly effective self-administered mental training, capable of inducing measurable neuropsychological transformation. The evidence not only lays the groundwork for its integration into psychological health frameworks but also validates its status as an integral human development system. More longitudinal research is required to establish causal trajectories and explore the role of advanced methods and the philosophical-motivational context.
